Subject: Fatal Crash Involving Fire Engine in Tennessee

Paulette,TN: An Anderson County man was killed Saturday when he veered into the path of an oncoming fire engine that was responding emergency traffic to
a chimney fire.

First Responders and an Emergency Medical Technician onboard the engine immediately began care on the 35 year old man, but he was killed on impact when he struck the 1986 Mack pumper head-on.

Ironically, the engine involved in the accident was the truck that replaced another truck that rolled over in June 2002 while responding to a brush fire. In that accident, the driver of the tanker/pumper was airlifted the University of Tennessee Medical Center for severe neck and back injuries. He made a full recovery. What a bad first day driving.  Medic 7 was almost totaled when hit by a drunk driver, while transporting a priority 1 patient to the hospital.  The driver of Medic 7 was fresh out of the Academy, his first night driving.  Luckily no one was seriously injured.

A Calaveras County fire truck that was temporarily stationed in Lucerne Valley responded to a fire in Lytle Creek on 8-11-02. The truck was traveling south in I-15 when a few hundred feet past the Hwy-138 overpass when the driver suddenly lost control. The truck was loaded with 500 gallons of water and 3 crew when it went suddenly from the #4 lane over the 30-foot embankment. The CDF has grounded thier fleet of trucks as the crashed truck only had 3600 miles on it. On 8-13-02 the pieces were loaded on a truck and hauled to a secure CHP site for further investigation. The crew was said to be recovering from the high speed crash.

On the evening of May 20, 2003 Cincinnati F.D. Ambulance 17 was involved in this accident.  The unit was travelling south on Central Parkway nearing the intersection of Marshall Avenue when the vehicle left the rain coated road and struck a parking sign.

Shown here is the ambulance along with District 3 who was called to the scene to make a report.  There were no injuries to the occupants, however the sign post did not survive.

Pierce Demo Aerial Crash on Colorado Interstate
« Reply #7 on: September 09, 2003, 11:38:02 PM »
Story by Annie P. Hundley, Greeley Tribune

JOHNSTOWN, COLORADO  — A new fire truck was destroyed Thursday afternoon when the vehicle rolled off Interstate 25 on the way to a sales demonstration in Fort Collins and backed up traffic through rush hour.
The driver, Marc Faluotico, 29, wasn’t seriously injured, but he was taken to McKee Medical Center in Loveland for observation. Faluotico, of Broomfield, is an engineer for the Westminster Fire Department.

He was working his other job as a salesman for Front Range Fire Apparatus, a fire truck dealer based in Boulder, when the crash occurred.

Faluotico lost control of the truck at 1:05 p.m. The truck rolled off the east side of the interstate at the offramp to Colo. 60. The offramp was closed and traffic was stalled until 6 p.m., according to Colorado State Patrol Trooper Brian Williams.

Officials on the scene said the crash may have been caused by a mechanical malfunction. No one else was in the truck.

Jim Wolfinger, a salesman for Pierce Manufacturing Inc. from Appleton, Wis., was trailing Faluotico in another fire truck on the way to Fort Collins. Pierce Manufacturing owns the $600,000 truck, which is insured.

“He said he lost control,” Wolfinger said. “He felt something go wrong.”

The Fort Collins Fire Department had expressed interest in buying a truck like the model that crashed. The truck has a 105-foot ladder, and the two salesmen were headed north to show the fire department what it could be getting. During the rollover, the front cab totally disconnected from the body and the truck had to be hauled away on a flatbed truck.

The truck was recently manufactured in Wisconsin, and had few miles on it beside the trip from Wisconsin to Colorado.

Traffic on northbound I-25 was backed up behind the accident, and several minor secondary crashes occurred. Williams said the traffic was made worse when many drivers cut through the median to drive on Frontage Road. Doing that is illegal, Williams said, and backs up traffic even more when cars merge back onto the highway later on.

Jonesboro, Arkansas Firefighters Injured in Head-On Firetruck Collision

From Firehouse.com (http://cms.firehouse.com/content/article/article.jsp?id=29698§ionId=45)

CHRIS MACKEY

Jonesboro Firefighter Tim Rook is in stable condition & recovering from Saturday’s firetruck accident.

On Saturday, April 24, Firefighter Tim Rook was responding to a commercial fire alarm in Squad 2, when as he approached an intersection, an 18-wheeler, gasoline tanker truck pulled out in front of the firetruck. The firetruck hit in the center of the tank trailer, pinning Rook in the cab. Firefighter Morgan Walling was thrown to the floor and sustained minor injuries.

As units responded to their brother, Firefighter Walling, with the help of off-duty firefighter Paul Pittman, attempted to extricate Rook, by using the portable Amkus unit from their very own truck.

Brother Rook received a severe head laceration and a compound fracture of his left Tib/Fib. He has had 3 surgery’s and several units of blood. A graft of a blood vessel had to be taken out of his right leg to repair the damaged one.

The Jonesboro Fire Department asks everyone to keep Rook in their prayers.

Bus collides with fire truck... [Buffalo]

Bus collides with fire truck raises financial questions

May 6, 2004

A fire truck responding to a suspected arson fire in Buffalo collided with a bus early this morning.

The accident is just the kind of thing that worries the district attorney, who is upset about plans to disband the city's fire investigation unit.

Three adults on the bus went to the hospital, as well as four firefighters.

“Two were back boarded. Two were hurt less severely but they also went to the hospital for evaluation,” said Chief Tom Rapp.

They were on their way to a suspected arson fire on Northland Avenue.

Under Mayor Masiello's cost-cutting budget proposal, the city's fire investigation unit will be disbanded, with the 9 members transferred to the firehouse level. That idea has the district attorney hot under the collar.

“I'm just saying right now, cut wherever you want, but when the tragedy hits and there's no one there that can investigate the problem, then everybody's going to say what went wrong,” said D.A. Frank Clark.

Fortunately, no one was killed in this morning's crash. But it's the 'what if's' that Clark is worried about. If there were fatalities, and there were no investigators to track down the arson suspect, then perhaps no one would've been held accountable for the accident.

Clark hopes for a county-wide arson investigation unit, avoiding a gap that could create more problems in the city.

Two firefighters were treated and released, two others are still being treated for neck and back injuries.

The three people on the bus, a 15-year-old student, the driver and a monitor are all said to be ok.

mvbn1, I will assume this is also yours, but please don't mind if I post it. It comes from 1strespondernews.com

TOLEDO, OH - Around noon on Monday, the Toledo Fire Department dispatched Engines 25 and 17 on an EMS run. Within minutes, Engine 17 was advising dispatch, that they had been involved in an accident at Central and Jackman. The engine was headed west on Central, and when they approached Jackman the engine went left of center, to pass traffic at a light. It appears that a car turned left from Jackman onto Central, in the path of the engine. It looked as if the operator of Engine 17 was attempting to avoid hitting the car head-on, and ended up hitting a pick-up truck, which had pulled over yielding to the fire apparatus.

West Virginia Firetruck Crashes Into House

West Virginia Firetruck Crashes Into House While En Route To Call

MICHELLE BLUM

WHEELING - Gene Ryan stood in the middle of Lind Street Sunday evening watching as a heavy-duty tow truck pulled a Wheeling Fire Department engine from the front of his house.

"Well, I'm going to need a new porch and a new garage door," Ryan said.

Ryan, who has lived in the two-story frame home for the past nine years, had replaced the porch two years ago.

Engine 2 was responding to a call of a tripped fire alarm in a building further up on Lind Street at the time of the incident, about 4 p.m. Sunday, Assistant Fire Chief Tom Wilson said.

The engine was traveling east on 14th Street and had to veer slightly to the left to enter Lind Street. But whether it was human error or mechanical malfunction that caused the driver to lose control of the engine, Wilson couldn't say. But suddenly, the truck bearing three firefighters struck the house and became lodged beneath the porch.

None of the yet-to-be-identified firefighters were injured. Ryan and his wife were not home at the time of the incident, Wilson said.

Two dogs and two cats that were in the residence were not injured, Ryan said.

Ryan and several family members were on their way to the Upper Ohio Valley Italian Festival and saw engine 2 heading up 14th Street. A short time later, they noticed a large group of people standing in front of his house.

"I came home and found this," he said.

It took more than four hours to free the engine from the house. Finally, just after 8 p.m, with a heavy duty tow truck from Fleet Service of South Wheeling pulling carefully, the porch creaked slightly as did crumbled metal from the fire engine. Work stopped briefly as pieces of Ryan's garage door were pulled from beneath the front bumper of the fire engine, where they had become lodged.

A few minutes later, the effort resumed and the engine was pulled free.

The roof of Ryan's porch was supported by a large beam but whether the residence was structurally sound hadn't been determined by late Sunday.

For a time, the engine, the driver's side of the windshield broken, its light bar gone, sat near the intersection of 14th, Wood, and Lind streets before it was removed from the scene.

Firefighters Battle Blaze on Own Truck [Providence, RI]

From the Associated Press...

Firefighters Battle Blaze on Own Truck
Tue Apr 19, 8:12 PM ET

PROVIDENCE, R.I. - Providence firefighters spent part of Tuesday morning trying to subdue a stubborn blaze — in their own fire truck.

Engine 11 was completely burned after a fire started in the engine compartment while the truck was driving in Roger Williams Park.

The engine's crew tried fruitlessly to fight the flames with fire extinguishers. They had to call in another truck to put out the fire.

"This is unusual," Capt. Peter Celini told WPRO-AM. "I've been here a long time, and I've never seen a fire truck fully involved like that."

One firefighter twisted his ankle and was taken to a hospital, but no other injuries were reported.

There was no word Tuesday from the fire department on a possible cause.

Fatality Reported In Car/Fire Truck Crash
Several Firefighters Injured In Accident

POSTED: 10:21 a.m. PDT May 5, 2005
UPDATED: 11:16 a.m. PDT May 5, 2005

Story by nbc4.tv

LOS ANGELES -- Several firefighters were injured and a civilian was killed in a crash at Jefferson and Budlong streets in the North University Park area, northwest of the University of Southern California campus.

Helicopter video from the scene shortly after 10 a.m. showed that the truck crashed into a tree and at least three cars. One of the vehicles was lodged under the front of the fire truck after the accident.

A Fire Department spokesman said there were three people in a car that was hit, and one of those people was killed on impact. The others were injured and were reproted in moderate to serious condition.

Four firefighters were also injured. They were all reported in moderate to serious condition. Eight injuries were reported in all.

Los Angeles City Fire Deputy Chief Andrew Fox said the fire truck was in route to a structure fire, with its lights and sirens on, when a motorist reportedly traveling at high speeds did not yield to the fire truck.
